I don't see anything out of the ordinary with the file.

Maybe it's something to do with the naming of the app. Try a different name and see if that makes a difference. BTW, ensure the ownership permissions are set to apache:users for the index.html file.

If it still does not work, post the permissions of the file. For exampme:
